A reminder . . . IIRC this was last mentioned casually in May 2003 followed
by a very detailed thread about the issue in July 2003.

**WARNING**
If you go with a Tamron Adaptall II lens for the OM-4 and pick up a used
Adaptall II adapter, *ensure* you have the newest version!!! One of the
four countersunk screws on the early Adaptall II's for the OM system
crosses over the top of the small black reset button found on the OM-3[ti]
and OM-4[T] lens mounting flanges. [Other early non-OM lenses also suffer
this same issue]. If it does, it will shear off the tip of the small black
reset button when the lens is mounted to an OM-3[ti] or OM-4[T,ti]
body. The earliest Adaptall II's are marked with an "OM" on the side
facing the camera body. These *all* have this problem and should **never**
be used with an OM-3[ti] or OM-4[T,ti].

Identifying Adaptall II's that are compatible with OM-3/4 bodies is more
difficult. Later ones are marked "OL" ***but*** . . . and this is a very
important "but" . . . I've been told some of the earliest "OL" Adaptall
II's also have the same problem as the "OM" version. If it's marked "OM"
it's definitely a problem. The newer ones marked "OL" require inspection
of their screw locations to ensure the countersunk screw nearest the
stop-down DoF preview lever will not cross over the top of the reset button
when the lens is mounted. It should be right next to the DoF preview lever
when the lever is in its normal "rest" position. That particular screw
location should be "asymmetric" compared to the other three which are
spaced 90 degrees apart from each other. If in doubt, hold the Adaptall II
near the camera lens mounting flange and rotate it from "red dot" to "red
dot" on the body (as if mounting it) while watching the screw location and
the black reset button.

NOTE:
The OM-1[n], OM-2[n,S,sp] and "double digit" bodies do not have this reset
button. It's not an issue with them. If those are the only bodies you
have and never anticipate having an OM-3[ti] or OM-4[T,ti], you have
nothing to worry about.
