There were many obstacles to a safe passage.  

The radio operators were paid by a separate company, were paid according to the 
number of private telegrams sent and therefore prioritised private messages.  

The rivets with which the hull was manufactured were mostly new steel ones, but 
these needed an automatic riveter which could not be used in some parts of the 
hull; therefore the older riveting system, with iron rivets, was used there.  
The resulting inconsistency of strength across the hull caused weakness.

The whole documentary, about the Titanic, which we used was pretty harrowing.  
But my partner and I used for our case study the failure of a surgical 
operation, an excellent example of human factors problems.  If you are 
interested in Human Factors, you could search for Elaine Bromiley on the 
Internet.  Her widower examined the performance of the surgical team and 
generated so much heat that changes took place after his campaign.
