True, if for no reason other than not using the correct equipment :-)

Yes, that helps as well, mostly to reduce the number of floating particles
between yourself and the subject.  You still need proper illumination though.
If there's no red's or yellows present, it doesn't matter how close you
are, the photo will still look blue :-)  Here are a few example of what good
lighting combined with close distances can produce:
