Things to consider:
Cleaning-
The OM:
Blow out the shoe socket on the OM with canned air. DO NOT use your
breath. DO NOT squirt anything down there.

The shoe 3:
Your post said it was NIB. But check the contacts if they are dirty you
can use a pencil eraser to clean them. Also closely look at the contact
points on the shoe make sure they are all at the same level in relation
to the shoe base. Reseat the shoe make sure that it is tightened all the
way down just don't use brute force.

The T32:
Examine the contacts on the bottom of the foot. Again you can use a
pencil eraser to clean them. Be careful though these are spring loaded
and you don't want to get bits of eraser caught in the them. On that
note make sure all three contacts on the foot move freely and spring out
to the same height. Also there is a thrysistor(?) in the "O" in Olympus
on the front of the T32. Check to see if it has dirt in it. I don't
think it has anything to do with OTF metering but it is there so check
it.

Put fresh batteries in everything and try the combination again.

Other suggestions:
Borrow the T20 from your mom. Try it in place of the T32 if it functions
normally then its probably the T32.
If the results are the same, then its either the shoe 3, a loose contact
in the socket or flaky electronics in the OM2.

One last possibility. Were you allowing enough time between shots for
the T32 to fully recharge? I've been guilty of not letting it recharge
fully and it will result in underexposed photos and/or the flash failing
to fire.
