My OM=>EOS adapter is a hand made job machined from a solid piece of
brass. Not a particularly elegant piece of work. The spacing on the part
that receives the OM mount was subtly off. Some lenses were a bit hard
to mount. A few moments with a genuine Oly piece and a file cured that.
The maker also erred in the method of imparting some spring in the ears
that hold the ears on the OM lens mount snug. Thin slits are cut in the
ears to provide a little give and tension. However, the brass is too
ductile and simply deforms after a bit with a heavy lens hanging off it.
I cured that by inserting some thin strips of aluminum in the slits.
Works fine now. I probably should have just bought a Kindai for twice
the price, but I was intrigued by the 'Bay listing and had to try it
out. It's been fun.

Anyway, the point is that it is certainly possible for a machinist to
make one from one piece of metal and one small screw to act as the
aperture stop down actuator (slightly radially misplaced too, I think,
but only shows up on 1-2 lenses I've tried). So if you are serious about
making some, I'd be happy to send pics or whatever else might help. I
realize you are targeting the 4/3 mount, but the problem is essentially
the same, so seeing someone else's version might help envision yours.
