C.H. didn't post a link to his own very nice macro flash creation which
although not twin macro could be a good alternative starting point:

Perhaps a very simple hotshoe mount put near the edge of a circular
translucent white disk such that a T20 could be mounted on it and flash
through the disk could allow the flash position to be changed, giving
the desired shadowing. One T20 could be mounted on the disk, a T32 could
sit on the camera hot shoe.

The flash brackets for the Arca Swiss style quick releases would be an
option but are incredibly expensive.
