I guess it could be something like that, occasionally an SD card will get
removed without doing an "eject", though why would these images just
reappear after 3 months after many uploads of images from the card ?

Did you use the "Safely Disconnect" procedure in Windows before unplugging
the camera or card reader, or removing the SD card?

To provide the best performance, Windows uses a disk cache. This is an area
of memory (RAM) that is used to retain recent changes to the disks.  Windows
writes these changes to the actual physical disks at some later time.  If
you remove the SD card before these changes are written to the card, the
changes will be missing (in your case, the deletions).

The "Safely Disconnect" procedure causes Windows to write the outstanding
changes to the disk, before telling you that you can now safely unplug the
device.

For USB disks (this includes SD cards), there is a Windows setting that you
can switch between "Maximum Performance" (uses the disk cache), and "Maximum
Safety" (does not use the disk cache, so performance is reduced).
