The full report will drill down to whether it was CRM PF/PM task
sequence error or a failure for TOGA to be engaged due to control
design issues. It was 12 seconds after where TOGA should have been
engaged and when it actually was.
Of course, it will always come down to pilot error, but why? All of
the other items in the go around procedure were followed, how did the
one most critical one get missed? Either the pilots assumed the other
got it or the one who was supposed to do it did not press the button
correctly and failed to monitor the change in throttle position.
Or as Edwin Musk would put it: "A procedural incongruity resulting in
the unapproved parking of the airframe and thermal damage."
