Your hand holding was well done but you're just up against the optical
limits. My guess is that this shot was taken from about 2 feet and, at
f/5.6 you're only going to get a couple inches DOF at best. A tripod
and f/8 would have given you another inch or so. f/11 would add another
inch or so but by then the diffraction boogeymen might start showing up.
