Some additional info I have found on Qualex:

Qualex was established in 1988 when Eastman Kodak and Fuqua
Industries, the predecessor of Actava and parent of photofinisher
Colorcraft Corp., announced the merger of their U.S. photofinishing
operations.  Since then, Qualex has become a wholely owned subsidiary
of Kodak.

The combined operation included 53 laboratories and brought
Colorcraft labs and Kodak-owned labs, including Ektra Photofinishing
(formerly Fox Photo), Qualis Photofinishing, American Photo and CX
Corp., all under the Qualex umbrella.  Since then, Qualex has been
closing plants to consoldate its operations.
  
Qualex is now doing ALL its processing for Kodak mailers in the
District Photo labs in Beltsville, MD. (I guess you would call this
outsourcing)  I believe there are other Qualex labs in operation
(perhaps about 13) to support digital printing.  District also owns
Clark Color Labs, Mystic Color Lab, and Snapfish.

If you have old mailers with Fairlawn NJ address, they should get
forwarded by the PO, but to be safe, I would address them with the
new address for the Beltsville location.  I got this address by
calling Qualex Customer Service 800-828-4587 (a number not easy to
find).  The people I talked to (I called a  couple of times to see if
I got the same answer, and I did) gave me the address below and also
assured me that Kodachrome is still processed there in Beltsville. 
The Kodak website link below gives a PO box number instead of the
street, but they should both get them there.
