Knob comes unscrewed when someone rewinds a roll of film in the wrong
direction, then they put it back together and most people put the spring in
the wrong way, thinking  the little square tab is supposed to push against
the handle. It doesn't, tab goes down towards camera, away from handle.

I can't see how it would fall apart without removing the camera top cover as
well... but this might be necessary to your fix.  I'd GUESS that either the
crank is stripped, or the peg that holds it is stripped, or the screw itself
is stripped.  Hopefully it is tha last of these, in which case it's time to
visit one of your parts bodies... Oh c'mon... you know most of us have them.
  Sometimes one OM has to pass away to keep many others alive... lol.

Of course, you could always just use it with a winder... ?
