One other problem which _may_ be related to your problems is that some
of the OM-2s's have a weak spring in the take-up spool's clutch. You
can check by loading and winding through a roll of film with the back
open -- the film on the take-up spool should stay wrapped somewhat
snugly around it through the whole roll. If the film becomes loose, so
that it fills the take-up chamber, the clutch spring (inside the spool)
should be replaced with a tighter one.

Another unlikely possiblity is that the rewind shaft may not be exactly
perpendicular to the body, though the usual complaint with this is that
the camera gets difficult to wind after several frames -- if the shaft
is askew, it won't turn freely, and once the slack inside the cassette
is taken up, it takes greater torque to pull the film from the cassette.
