There is more factor, though - the development environment...

Many development tools are optimised for one or another browser family.
An ancient example was M$ Frontpage, that produced code that
(naturally!) worked best with IE, and barely worked in some other
browsers. Nowadays it's the dynamic content management software with
cascading style sheets. The developers provide the CSS so the 'look and
feel' of the page is preserved, unfortunately in many cases the CSS is a
bug-ridden abomination. 

I still use an old (Presto) version of Opera for some things.  It was
always fun to open the error console, and watch it spew a fountain of
errors from the CSS on slow or non-working pages.  :)  

Opera in those days was always 100% w3c compliant, which none of the
other big-name browsers were. Interestingly IE11 is now.  Chrome (and
current Opera which uses Chromium engine) is only 99% compliant, and
firefox only rates 98% compliance. (tested with the ancient
acid3.acidtests.org test.)
