Oh I've managed that - more experienced heads here will offer other  
causes I'm sure but here's my stupidities -
1. A bang on the side can stress some element seating rings  
momentarily and cause small edge fracture in the soft glass,  
especially what's known as 'shells' in leadlighting and Tiffany work.
2. Dismantling a lens and then pushing the elements back in clumsily  
- sometimes they are a tight fit and hard to seat properly.
