It sure sounds like John has the right idea ... the switch isn't being
pressed in by the adapter.
-Perhaps the camera mount itself is slightly distorted (zuikos harder to mount)
-Distortion of adapter near preview button

The tab in the camera body that the lens lock presses against may have
become rounded, allowing the lens to be unlocked without pressing the
unlock button. The small tab in the adapter may also be slightly
rounded. Replacing the adapter may solve both problems.
